Affidavit

It wasn’t possible to activate your DSL connection two weeks ago. The technicians from Deutsche Telekom informed us that this was because you missed your scheduled appointment. Deutsche Telekom has charged us for the unsuccessful visit.

If you were at home despite the telecom technician’s statement, then we kindly request you to complete a sworn statement and send it to us. Therefore, you need to mail us again to request the affidavit in English.
We will not pay Deutsche Telekom for the unsuccessful visit based on this statement.

Note that the time specified on the sworn statement is 8 am to 1 pm. You may have already received our message that the technician wasn’t coming before 1 pm. If you were no longer at home afterwards, then please correct the time on the sent form.

If you don’t return the document to us within ten days then we have to assume that you weren’t actually at home. In this case we will charge you €48.79 (VAT included) for the next appointment in line with our general terms and conditions.
